Self-directed support services – putting you in charge

What is Self-Directed Support?

Self-directed support is about choice. It means giving you control of what kind of social care you want and who you want to get it from.

Users of self-directed support receive a personal budget to spend on their social care needs. You then draw up a support plan to decide how best to spend your money.

How we can help

If you receive a personal budget, then Health Works can offer you support.

We offer a personalised, flexible service tailored to meet your needs. If you would like to find out more, please call us to arrange a free initial meeting where we can discuss your individual requirements and create a package that works for you.

Why choose Health Works?

With Health Works, you can:

Services

Any one of these services can be offered individually or combined into a package at no additional cost. For example, you might want someone both to befriend you and to accompany you to activities as a buddy. The basic rate would stay the same for however many hours you require.

Our services are tailored to suit you. You can arrange for your support worker to meet you wherever you need them, and we guarantee to provide cover if and when required.

Befriender / Buddy

£8-£15 per hour depending on level of support required

A befriender is there to support and encourage you to help reduce isolation. For example, they can come to your house for a cup of tea and a chat, help you with the shopping or go out for a walk.

A buddy supports you to access activities e.g. going to the gym or a community group.

Support Planning

£15 per hour

We can help you to create your support plan for your personal budget. We can signpost you to organisations offering the help you need, and suggest ways in which you can find support as indicated in your assessment questionnaire. Once your support plan is in place, we are available to help you amend it when necessary and signpost you to other relevant organisations.

Advocacy

£15 per hour

An advocate works on your behalf to make sure that you are receiving value for money in the services that you buy using your personal budget, and to advocate on your behalf with your service providers. They can also help you to review your support plan.

Money management support

£15 per hour

Do you need support to manage your personal budget? We can provide you with advice and assistance as and when you need it. We can also work with you one-to-one to develop your money management skills so that you can learn how to manage your budget yourself.

Budget holding support

£15 per hour

We can manage your budget for you so that you don’t have to worry about it.

This service can include: holding your budget, organising monthly outgoings, paying invoices, paying PAs, keeping accounts and preparing for audits. We will keep you up-to-date with your budget on a regular basis so that you always know what’s going on.
